Free T3 is the active hormone your cells actually use, reverse T3 shows whether stress or illness is blocking conversion, and antibodies identify Hashimoto's, the autoimmune condition behind a large share of hypothyroidism. Ferritin, vitamin D, B12 and selenium are checked alongside, because the thyroid cannot work properly without them.

How It Gets Treated

Treatment follows the panel. Where thyroid support is warranted it is dosed against free T3 and free T4 as well as TSH, and re-tested. Where antibodies are present, the conversation widens to the immune and inflammatory drivers behind them, because medicating the gland without addressing what is attacking it leaves half the problem in place.

Why do I have thyroid symptoms if my TSH is normal?

TSH is a pituitary signal, not a measurement of the hormone your cells receive. Free T3 can be low, or reverse T3 elevated, while TSH still reads inside range. A full panel is the only way to see that.

What is Hashimoto's thyroiditis?

It is an autoimmune condition in which the immune system attacks the thyroid, and it accounts for a large proportion of hypothyroidism. Antibody testing identifies it, and it frequently exists for years before TSH moves.

Can thyroid problems cause weight gain?

An underactive thyroid slows metabolic rate, so weight gain and difficulty losing weight are common. It is one of the first things worth ruling out when weight resists a genuine effort.
